What actually goes wrong
Apply to the right country
It's the country of your main destination, or of first entry when the stay is evenly split. Applying at the wrong consulate wastes the appointment slot entirely.
Insurance is checked, not skimmed
The certificate must name you, state a minimum of €30,000, and say it's valid throughout the Schengen area for the full travel window. Anything vaguer gets refused at the counter.
Order matters at the counter
Most VFS and consulate desks work through the list top to bottom. A bundle in their order gets accepted in one pass; a shuffled folder gets handed back.
