The order is the other way round from most Indian banks: the date comes first, and it is 3 letters, not 4. Equitas's second example is a name with initials, D.J.Durgaprasad born 20 January, which becomes 2001DJD. This rule is on the bank's mobile banking page. We found no published rule for Equitas credit card statements. The email your statement came in states the rule for your account, so read it if the format above does not open your file. If you never received a password rule at all, only Equitas can send the statement again.

Then check that you used your name exactly as the bank has it, that the date is day first and month second, and that you used the first holder's details on a joint account.
